Youβll work within the RIACT team, supporting safe and timely discharges from hospital and ensuring individuals receive the right level of care post-discharge.

π Key Responsibilities
* Complete strengths-based assessments to support discharge planning
* Facilitate safe and timely hospital discharges
* Arrange intermediate care and longer-term support services
* Carry out post-discharge reviews to assess ongoing needs
* Work both independently and as part of a multidisciplinary team
* Apply knowledge of the Care Act 2014
* Maintain accurate and high-quality case records

* Qualified Social Worker
* Experience within Adult Social Care / Hospital Discharge / Community Teams
* Strong understanding of Care Act and discharge pathways
* Ability to work independently in a fast-paced environment
* Excellent assessment and organisational skills
