Social Worker | Children in Care & Leaving Care Team | Aylesbury | £38 per Hour
Caritas Recruitment is recruiting for two Social Worker vacancies within Buckinghamshire's Children in Care and Leaving Care Service, based from the Aylesbury office.
This role focuses on supporting young people aged 16+ as they prepare for independence and transition into adulthood. The service is committed to ensuring young people receive the guidance, support and opportunities they need to achieve positive outcomes.
Key Responsibilities:
Manage a caseload of children in care and care leavers aged 16+.
Develop pathway plans and support young people towards independent living.
Work closely with young people to identify strengths, aspirations and future goals.
Support access to education, training, employment, housing and health services.
Maintain effective partnership working with internal and external agencies.
Undertake statutory visits, reviews and assessments in line with legislation.
Requirements:
Social Work England Registration.
Experience working within Children in Care, Looked After Children or Leaving Care Services.
Knowledge of relevant legislation relating to care leavers and children in care.
Strong relationship-building and advocacy skills.
Commitment to improving outcomes for young people transitioning to adulthood.
