You’ll play a key part in launching new care packages, supporting care teams and making sure individuals and families receive consistent, personalised support. With an experienced leadership team behind you and opportunities to progress, this is a chance to put your knowledge into practice and have a real say in how services develop.
Candidates with three years’ relevant experience within complex care or community care services will be considered where they can demonstrate the skills and readiness to step into the role.

* £35,000–£40,000 salary, depending on experience.
* Full-time, permanent employment.
* Paid mileage for client visits and a company pension.
* Employee referral scheme.
* Ongoing training and support towards further qualifications.
* Guidance from an experienced complex care leadership team.
* Opportunities to progress into regional or strategic leadership.
* The opportunity to help develop services and see the difference your work makes.
Your role
Combining day-to-day service oversight with hands-on team leadership, you will:
* Coordinate the launch of new complex care packages and oversee existing services.
* Visit individuals and families, building relationships and ensuring care reflects their needs.
* Review and sign off care plans in collaboration with clinicians and the wider multidisciplinary team.
* Liaise with ICBs, local authorities and other professionals, preparing reports for commissioners.
* Support and develop care teams through supervision, appraisals and team meetings.
* Help manage rotas, resolve service issues and maintain continuity of care.
* Participate in the on-call rota.
We’re looking for:
* Relevant experience in complex care or community care services. Three years’ experience will be considered for the right candidate.
* Experience coordinating care packages, with an understanding of complex care delivery for adults and children.
* Knowledge of needs such as ventilation, tracheostomies, spinal injuries, acquired brain injuries and bowel care.
* The ability to lead, support and motivate care teams.
* A sound understanding of care quality and regulatory requirements.
* Confidence communicating with individuals, families, clinicians and commissioners.
* A driving licence and access to your own vehicle for regional travel.
* Flexibility to meet service needs, including occasional early mornings, evenings or weekends and on-call responsibilities.
