We're recruiting for a Complex Care Case Manager to join an established specialist care provider, overseeing community-based care packages across Merseyside.

As a Complex Care Case Manager, you'll oversee the delivery of high-quality care packages for individuals with complex healthcare needs across Merseyside.

Your responsibilities will include:

Managing a caseload of community-based complex care packages
Completing care assessments, care plans and risk assessments
Coordinating and mobilising new care packages
Leading, supervising and developing community care teams
Building strong relationships with clients, families and healthcare professionals
Ensuring compliance with CQC standards and safeguarding requirements
Monitoring care quality and driving continuous improvementAbout You

I'm keen to hear from experienced Care Managers, Complex Care Coordinators, Case Managers and Field Care Supervisors who are confident managing care packages in the community.

You'll ideally have:

Experience coordinating or managing complex care packages
Knowledge of neurological conditions, physical disabilities or long-term complex healthcare needs
Experience supervising and supporting care teams
Good understanding of CQC regulations and safeguarding
Strong organisational, communication and leadership skills
