About the role
As our Community Care Manager, you'll lead the delivery of safe, person-centred care by building strong relationships with customers, families and healthcare professionals. You'll oversee customer assessments, care planning and reviews while ensuring every service is compliant, effective and tailored to individual needs.You'll drive quality through audits, spot checks, investigations, safeguarding and continuous improvement, helping us maintain the highest standards of care. You'll also provide leadership and act as a key link between the Registered Manager and Supervisors, providing leadership across the field team, support operational decision-making, participate in the on-call rota and provide emergency cover when required to ensure continuity of care.This is an exciting opportunity to influence how care is delivered, champion best practice and make a genuine difference to the lives of our customers and colleagues every day.If you're passionate about quality, thrive in a fast-paced environment and want to help shape the future of an ambitious, growing care provider, we'd love to hear from you.
