Location: Wolverhampton (office-based with regional travel across the West Midlands & Shropshire)
Salary: £33,000 – £35,000 per annum (depending on experience)
About the Role
We are looking for a dedicated Complex Care Manager to lead and oversee the safe delivery of community-based complex care packages for both adults and children across the West Midlands and Shropshire. You will play a vital role in ensuring our clients receive consistent, person-centred, and compliant care in their own homes.
This position is office-based in Wolverhampton, with regular travel to service users and commissioners. You will work closely with clinicians, ICBs/CCGs, local authorities, families, and our wider MDT, ensuring the highest standards of service and safety.
Key Responsibilities
* Set up and coordinate new care packages.
* Maintain existing services, ensuring high standards of care and compliance.
* Visit clients regularly, ensuring person-centred care is always delivered.
* Liaise with clinicians and MDTs to review complex care packages.
* Meet with local authorities, ICBs/CCGs, and other professionals.
* Complete and sign off care plans in line with regulatory requirements.
* Prepare and submit reports to commissioners as required.
* Provide leadership, supervision, and support to care teams.
* Cover emergency shifts where necessary.
Requirements
* Minimum 5 years’ experience in community or domiciliary care.
* Experience in complex care (ventilation, spinal injury, tracheostomy, acquired brain injury, bowel care, etc.) is essential.
* Proven experience working with both children and adults in complex settings.
* Strong leadership and client-facing skills.
* Driver with access to a vehicle (essential).
* Ability to work flexibly – some packages may require early mornings, evenings, or weekends.
* Office-based in Wolverhampton with regional travel.
