About the Role
We are looking for an experienced and dedicated Care Coordinator to join our team in Bristol.
The successful candidate will play a key role in coordinating the delivery of high-quality care and support services, ensuring that service users receive safe, consistent and person-centred care. You will work closely with care staff, service users, families, healthcare professionals and management to ensure the smooth running of our care services.
Key Responsibilities
Coordinate and organise care packages and staff rotas.
Allocate care staff to service users based on their needs, skills and availability.
Ensure care visits are appropriately planned and covered.
Maintain regular communication with service users, families and care staff.
Respond promptly to changes, cancellations and staffing requirements.
Monitor the quality and continuity of care being provided.
Assist with the recruitment, induction and onboarding of care staff.
Support staff with day-to-day queries and operational issues.
Maintain accurate and up-to-date care records and documentation.
Ensure care plans and risk assessments are reviewed and updated as required.
Support compliance with company policies, procedures and relevant legislation.
Liaise with healthcare professionals and other external agencies when required.
Participate in audits, reviews and quality improvement activities.
Escalate safeguarding, health and safety or care concerns appropriately.
Provide out-of-hours support as part of the agreed rota, where required.
About You
We are looking for someone who is organised, reliable and confident working in a busy care environment.
You should have:

Previous experience in a care coordination, senior care or similar role.

Good knowledge of health and social care services.

Excellent communication and interpersonal skills.

Strong organisational and time-management skills.

The ability to prioritise tasks and work under pressure.

Good attention to detail and accurate record-keeping skills.
*
A professional and compassionate approach.
*
The ability to work independently as well as part of a team.

Good IT and administrative skills.
*
A commitment to providing high-quality, person-centred care.
Desirable:

Experience working within domiciliary care or supported living.
*
Knowledge of CQC requirements and relevant health and social care legislation.

Relevant health and social care qualification.
*
Experience using care management/rostering software.
*
Previous experience supervising or coordinating care staff.
