The organisation is entering a period of growth and is looking for a motivated professional to support service delivery, workforce planning and package mobilisation.
Whether you are currently working as a Scheduler, Care Planner, Field Care Supervisor or Senior Coordinator, this role offers the opportunity to develop within a supportive and ambitious team.
Package & Benefits
£28,000 – £32,000 per annum
Monday to Friday, 9:00am – 5:00pm

As a Care Coordinator, you will play a key role in ensuring safe, effective and person-centred care delivery across community-based services.
Responsibilities include:
Coordinating staff rotas and care schedules
Managing planned and emergency shift cover
Supporting the mobilisation of new care packages
Building positive relationships with service users, families and care staff
Liaising with healthcare professionals, commissioners and external stakeholders
Supporting recruitment, onboarding and staff inductions
Maintaining accurate records using care management systems
Assisting with compliance, quality assurance and safeguarding processes
Supporting the Registered Manager with the day-to-day running of the serviceAbout You
To be successful in this role, you will have:
Previous experience as a Care Coordinator, Scheduler, Care Planner, Rota Coordinator, Field Care Supervisor or similar role within domiciliary care, home care or complex care
Experience managing staff rotas and coordinating care packages
Excellent organisational, communication and problem-solving skills
The ability to work effectively in a fast-paced environment while maintaining attention to detail
A good understanding of safeguarding, compliance and person-centred care
Strong relationship-building skills with staff, service users, families and healthcare professionals
A full UK driving licence is preferredWhy Apply?
