We’re on the lookout for an experienced Care Coordinator to join a well-respected provider, C4 Care, delivering domiciliary, complex care and outreach services across Chester. This role isn’t for beginners. You must have at least two years’ experience as a Care Coordinator within domiciliary or complex care. Hands-on care experience is a bonus, but if you’ve only worked as a Care Assistant or Support Worker without coordinating experience, this one won’t be the right fit.

In this role, you’ll be at the heart of the operation, planning and coordinating care visits to make sure every service user receives the right support at the right time. You’ll build strong relationships with clients, families and your care team, lead and support staff to deliver high-quality care, and stay on top of everything from care plans and risk assessments to reviews and compliance. No two days are the same, and you’ll be the person keeping everything running smoothly, even when things change at short notice.
We’re looking for someone organised, confident and genuinely passionate about care. You’ll need to be comfortable using scheduling systems, handling admin, and communicating with a wide range of people. A full UK driving licence and access to a vehicle is essential, as you’ll be out and about completing checks when needed. Very occasionally, you may need to step in and support care delivery, so a hands-on mindset is important.
