Belmont Recruitment are currently looking for a Care Coordinator to join Walsall Council on an initial 3-6 month temporary contract. This is a full-time role working 37 hours per week, Monday to Friday.
Key Responsibilities
* Contribute to support planning and assessments for vulnerable adults and their carers.
* Support citizens to make their own decisions, exercise choice and promote independence.
* Assist with assessing individual needs in line with self-directed support and single assessment principles.
* Support citizens to promote their health, wellbeing, choice and control, with consideration of mental capacity.
* Undertake allocated care management duties.
* Communicate and engage effectively with service users, families and carers using a person-centred approach.
* Work closely with colleagues across the multi-disciplinary team and other community agencies.
* Help individuals access community resources and develop the skills and support needed to live independently.
Requirements:
* Experience of working with vulnerable adults in a community-based setting
* Full UK Drivers Licence
