South London and Maudsley NHS Foundation Trust is seeking experienced mental health professionals to join the team as Band 6 Care Coordinators across a range of services and locations. These roles are ideal for practitioners with previous experience working in mental health settings, who are confident in independently undertaking assessments, care planning, and care coordination.

What you can add:

* Assess, plan, implement, and monitor care in collaboration with service users, carers, and the multidisciplinary team.

* Deliver person-centred care that meets mental, physical, psychological, social, and spiritual needs, respecting diversity and equality.

* Administer medication safely, with knowledge of contraindications and side effects, and provide clear, understandable information to service users.

* Complete needs and risk assessments, ensure they are documented and actioned, and monitor commissioned care packages as required.

* Provide ongoing review of individualised care plans, promoting health, recovery, and wellbeing.

* Work at all times within professional, legal, and ethical frameworks, including relevant codes of conduct, safeguarding procedures, the Mental Health Act, CPA, and Trust policies, supporting service user choice where appropriate.

What you’ll need:

* Qualified healthcare professional with current professional registration in Social Work (SWE) or Occupational Therapy (HCPC).

* Evidence of ongoing continuing professional development (CPD).

* Substantial experience working in community mental health services (e.g. Home Treatment Teams, CMHTs) and/or acute inpatient settings.

* Strong knowledge of evidence-based interventions for individuals with complex mental health needs.

* Proven experience in completing comprehensive assessments, formulation, and care planning.
