Care Coordinator – Domiciliary & Complex Care

To be considered for this role, you must have:
A minimum of 1 years' experience as a Care Coordinator within domiciliary care and/or complex care. - This point is mandatory. Please DO NOT apply if you don't have direct care co experience as advised. Your application WILL NOT be considered.
Experience managing care rotas, care packages, and staff deployment.
A full UK driving licence and access to your own vehicle.
Full authorisation to legally work in the UK.
Minimum of a Level 3 in a Health & Social Care certification
Unfortunately, applications from Care Assistants or Support Workers without Care Coordinator experience will not be considered.

The Role
As Care Coordinator, you will be responsible for managing care delivery across a portfolio of service users, ensuring care visits are staffed appropriately and delivered to the highest standards.
You will work closely with service users, families, care staff, healthcare professionals, and management to maintain excellent outcomes and service quality.
Key Responsibilities
* Coordinate and manage care schedules and staff rotas.
* Match care staff to service users based on skills, experience, and compatibility.
* Conduct care reviews, spot checks, risk assessments, and service user visits.
* Support the onboarding and supervision of care staff.
* Ensure continuity of care and respond to changes in service user needs.
* Assist with emergency staffing cover and last-minute schedule changes.
* Maintain accurate records and ensure compliance with CQC regulations.
* Work collaboratively with the management team to support service growth.
About You
You will be an organised and proactive individual who thrives in a fast-paced care environment.
You will have:
* Previous experience as a Care Coordinator within domiciliary or complex care.
* Level 3 or above within Health & Social Care.
* Strong knowledge of CQC regulations and care standards.
* Excellent communication and relationship-building skills.
* Experience using electronic rostering and care management systems.
* The ability to prioritise workloads and solve problems under pressure.
* Confidence in conducting service user reviews, staff supervision, and quality monitoring.

SPONSORSHIP ASSESSMENT
Where sponsorship may be available for the role, sponsorship is not guaranteed and will only be considered for candidates who meet the Company's requirements and demonstrate the necessary competency, performance, conduct and suitability for the role.

Candidates who are already in the UK on a Skilled Worker visa and are legally able to make a qualifying switch to a new sponsored role may, where permitted under the applicable immigration rules, be eligible for sponsorship without completing the full three-month assessment period.
Completion of an assessment period does not create an entitlement or guarantee of sponsorship or a Certificate of Sponsorship (CoS). Following the assessment, the Company will determine whether the candidate meets the required standard and whether sponsorship is appropriate and available. Any decision to issue a CoS will remain subject to the candidate meeting all applicable UK immigration and sponsorship requirements, as well as the Company’s own sponsorship requirements.
