Wakefield / Salary: £25,000 - £26,000 DOE / Hours: 37.5 per week,
Monday-Friday, 9am-5pm
Additional: Some on‑call duties & occasional care cover

The Role of Care Coordinator
We're looking for an organised, proactive Care Coordinator who is passionate about delivering excellent care and supporting a growing service. This is a varied role where no two days are the same.
Your responsibilities will include:
Carrying out client assessments and creating detailed care plans
Completing rotas, spot checks, and audits
Ensuring full compliance with company policies and regulatory standards
Supporting marketing and business growth activities
Providing on‑call support on a rota basis
Covering care calls when required
Working closely with staff to maintain high standards of care and communication
What We're Looking For in our Care Coordinator
Experience working in the care sector
Strong organisational and communication skills
Ability to work independently and manage your own workload
A proactive approach to problem‑solving and service improvement
Confidence in leading and supporting a team
A full UK driving licence and access to a vehicle (essential)
Flexibility to support on‑call and occasional care cover
