Job Description
Trident Healthcare Solutions is seeking an experienced Band 6 Physiotherapist to join an Intermediate Care service based in Hull. The successful candidate will support discharge pathways (including DT2A) and manage a varied caseload across multiple community sites and patient homes. A car is essential for this role.
Responsibilities
• Provide high‑quality physiotherapy within Intermediate Care settings
• Support safe and effective discharge planning, including DT2A pathways
• Manage a caseload across community sites and patient homes
• Complete timely and accurate clinical documentation
• Work collaboratively within a multidisciplinary team
• Utilise SystmOne (desirable) for clinical record‑keeping
Skills and Experience Required
• Degree‑qualified Physiotherapist
• Experience within Intermediate Care, community rehab, or discharge services
• Strong clinical reasoning and caseload management skills
• Excellent communication skills
Essential Requirements
• Previous NHS experience
• HCPC registration
• Car driver with access to own vehicle
• Experience in discharge pathways / DT2A preferred
