Locum Reablement Assessor
Community-Based Service, 37 Hours Per Week, Flexible Shift Pattern
* We are currently seeking an experienced and motivated Locum Reablement Assessor to join a busy community-based service supporting individuals to regain independence and achieve positive outcomes following hospital discharge or within the community.
* This is an excellent opportunity for someone with a strong background in adult social care, reablement, or short-term support pathways who is confident working autonomously within a fast-paced multidisciplinary environment.
The Role
* As a Reablement Assessor, you will deliver strengths-based assessments and coordinate short-term, outcome-focused interventions designed to maximise independence, recovery, and wellbeing.
* You will work closely with individuals, families, therapy teams, health professionals, and community partners to identify needs, develop reablement plans, manage risk, and ensure effective support is in place.
Key Responsibilities
* Undertake strengths based assessments and reviews to identify needs and desired outcomes
* Develop and coordinate person-centred reablement plans promoting independence and recovery
* Provide advice, guidance, and signposting to community and statutory services
* Manage risk and safeguarding concerns using Making Safeguarding Personal principles
* Work collaboratively with therapy teams, health partners, and internal colleagues
* Support hospital discharge processes, equipment provision, and referral pathways
* Maintain accurate and timely records in line with service standards
* Contribute to service improvement and performance targets
* Provide guidance and support to frontline staff, role modelling best practce
Requirements
* Minimum Level 3 Diploma/NVQ in Adult Social Care (or equivalent)
* Previous experience at Senior Carer, Team Leader, or equivalent level
* Demonstrable assessment experience within health or adult social care settings
* Experience within reablement services or short-term pathways is highly desirable
* Strong understanding of strengths-based practice, risk management, and safeguarding
* Ability to manage a dynamic caseload and work independently
* Excellent communication and partnership-working skills
